SAFETY BULLETIN

1.      During tie in operations, never operate, clean, repair, service; including greasing; or adjust any valve or equipment or perform any operation on a block valve that is isolating the tie in operation.

 

2.      Never operate, clean, repair, service; including greasing; or adjust any valve or equipment that has been tagged out of service under the Lock Out-tag Out Procedure on the Gas Transmission web site. This applies to both hot and cold tie ins.

 

3.      Prior to starting any and all tie in operations, the pressure in the pipe being worked on must be secured at a safe level. If the pipeline pressure is too high to perform a safe tie in; the appropriate actions, including greasing of valves, should be completed and valves locked out of service before any tie in operations begin. Other than operating the fire control stack, never perform any operation that could affect the pipeline pressure during a tie in.

 

4.      Whenever possible move the controls, manometer and associated operations for the fire control stack up and out of the tie in hole to allow more room in the hole and to also allow the stack operator a more secure and safe location to perform those operations.

 

5.      Prior to starting any tie in operations, access into and out of the tie in hole should always be covered during the morning safety meeting. Adequate provisions should be made prior to starting any tie in operation to provide safe access and egress to the tie in hole.

 

6.      When operation of the fire control stack is required to be in the tie in hole, the operator of the fire control stack should use a face shield, when in close proximity to the stack, to provide an additional PPE in the event of a flash fire.

 

 

THESE PROVISIONS OUTLINED ABOVE SHALL BE CONSIDERED HARD AND FAST RULES AND SHOULD NEVER BE VIOLATED DURING ANY TIE IN OPERATION.
