Report Near Misses
The union recognizes that some employees worry about retaliation for reporting accidents, safety violations and near misses to their supervisors.  Because workplace safety is so important, the union is offering an anonymous way for employees to initiate safety investigations.   Use the form below to report safety complaints and concerns. The completed form will be emailed anonymously to the Union President.  Responsible management and union officers will investigate the accident, near miss or safety complaint.  The union will notify the membership of the outcome via the "Accident & Near Misses" link found above.  NO names will be used on the "Accident & Near Misses" report.

Please describe the near miss, accident or safety concern that you have in detail.  Be assured, NO names will be used in our report to the company or in our report on the web to the members   But we do need plenty of facts.  We need to know Who, What, Where, When and Why.  This is required.  We can't help unless we know where to begin the investigation, what to look for, who is involved, etc.
